Excursion Consent Form - Year 4 Pinball Excursion (Wednesday 2nd November and Thursday 3rd November)

Dear Parents and Carers

On Wednesday 2nd November (4J and 4C), and Thursday 3rd November (4H and 4T), we will be visiting Sunshine Coast Pinball (the excursion) as part of our Term 4 Design and Technologies unit. 

The aims of the excursion are to:

  • Support student learning by providing hands-on activities which will support their studies in the relevant Design and Tecnhologies unit
  • Provide students with an opportunity to observe a variety of arcade games, particularly pinball machines
  • Provide stimulus for students to then create their own pinball machines for assessment purposes
  • Provide students with expert knowledge

Excursion details:

Venue:

Sunshine Coast Pinball, located in: C-Square (Address: 52/64 Currie St, Nambour QLD 4560)

Dates:

Wednesday 2nd November, 2022 (4J and 4C)

Thursday 3rd November, 2022 (4H and 4T)

Times:

Each day, the bus will depart Kuluin State School at 9:00am, to arrive at Sunshine Coast Pinball at 9:30am. The bus will then depart Sunshine Coast Pinball at 11:00am, to arrive back at Kuluin State School at 11:30am.

We ask that students arrive promptly to school, prior to 8:45am, in order to leave Kuluin State School by 9:00am.

Supervision:

Whilst at Sunshine Coast Pinball, students will be under the supervision of Year 4 teachers, with staff from Sunshine Coast Pinball providing activity-specific information and safe-use of equipment. Students will move around the venue in small groups.

Equipment Requirements:

  • Full Kuluin School Uniform
  • Shoes – normal enclosed school shoes
  • Water Bottle
  • Morning Tea (Normal school lunch will be eaten after returning to school)

School Excursion Safety Brief:

On arrival, a Sunshine Coast Pinball staff member will speak to students and teachers collectively, to outline;

  • General venue rules
  • Safety Instructions
  • Discussion/briefing on forces and components involved with activities.

Risk Level:

Low- students will need to take the utmost care when entering, travelling on, and exiting the bus, and ensure they follow all safety guidelines provided to them at the venue. Students will be supervised by Kuluin State School staff while on the bus and at the venue.

Excursion costs:

The total cost of this excursion will be $18.40 per student. This includes bus transport and venue entry.  Payment options are listed in the consent form below.

  • I agree to and understand the refund policy as it applies to this excursion (see Excursion costs)
  • In the event of an accident or illness, school staff may obtain or administer any medical assistance or treatment the child/student may reasonably require, including contacting their doctor.
  • I accept liability for all reasonable costs incurred by the department in obtaining such medical assistance or treatment (including any transportation costs) and undertake to reimburse the department the full amount of those costs.
  • I have provided the school with all relevant details of the child/student’s medical or physical needs on registration/enrolment and where relevant have updated this information.
  • I give consent for child/student contact information to be shared in relation to this excursion in compliance with relevant Queensland Chief Health Officer’s Directions.
